Faculty & Instructor Request Forms

Submit requests on paper forms, electronically, or by FAX to (413) 577-0037. Please provide complete and accurate bibliographic citations for processing.

Short readings (single chapters, articles) should be placed on Electronic Reserve, due to limitations in space for printed materials and the demand for electronic versions of these materials from students.  Print reserves will continue to accept mongraphs, faculty personal copies of texts, lecture notes, along with problem sets that are kept in loose-leaf binders and added to throughout the semester.

Jan. 13, 2014: We are experiencing sporadic technical issues with Reserve request form submissions.  If you experience difficulties in submitting the request form, please contact Course Reserves at 413 545-2358 for assistance.
